A)
Linux Kernel Module Programming Guide
AUTHOR: Ori Pomerantz
Publisher: Iuniverse Inc
A good book with complete examples for kernl 2.x series

B) Linux Device Drivers
AUTHOR: Alessandro Rubini, Jonathan Corbet
Publisher: O'Reilly

If you look online you will get lots of info on this topic, here are my fav; one:

How to write kernel 2.6 module; Look for the latest version of "procreadwrite". This is a 2.6 kernel modules that demonstrates how to create /proc entires and write directly to user-land via tty.
