Default Directory index full!

Hello everyone
I don't know if this is a hardware or software (or both) problem so please forgive me for posting in the wrong place. I have 4 500GB Sata HD's configured as raid 5, and ext3 filesystem. I have been getting the following error lately, and it has caused the system to lock up. The server is Centos5 and the error is:
ext3_dx_add_entry: Directory index full!
From what I can figure out, this can be caused by too many zero-byte files, or too many files in a directory. How can I find out what is exactly causing the problem? df -i says that only 7% of the i-nodes are being used, and the raid is only at 44% full.

Take down server single user mode and unmount file system run fsck and see if you get any specific error.
