Default Postgresql logging

I am able to log all statements in postgreSQL, but also want to log page(php) name, the ddl/dml request is coming to postgreSQL.

Is there any way to accomplish that ?

You will get that data from webserver logs and not from pgsql logs as pgsql has no idea about php pages.

The ddl/dml logging has to be enabled from the postgresql.conf files which is located in the data directory of postgresql. For the php file name, as Vivek has correctly suggested, you will have to look into your web server logs.
